Medical Clowns Around the World

The concept of the medical clown originated in the latter half of the 20th century and was proposed by Michael Christensen, one of the founders of New York’s Big Apple Circus. In 1986, the Big Apple Circus Clown Care Unit established by him brought professional clowns into the pediatric healthcare system for the first time in the world. This model demonstrated that laughter can effectively divert children’s attention from pain and help ease the tension of their families, and it also influenced communities around the world.

From the 1990s to the beginning of the 21st century, this professional concept of healthcare rapidly spread worldwide. International organizations were established one after another, including France’s Le Rire Médecin, the Netherlands’ Cliniclowns, and Austria’s Red Noses International. Countries began establishing rigorous training systems, and today medical clown organizations exist across all five continents in more than 120 countries.

Today, medical clowns have become important members of hospitals and long-term care institutions in many countries around the world. They are not merely performers, but professionally trained arts practitioners who collaborate with medical teams and use music, improvisation, and interaction to create emotional support and psychological relief for patients, families, and healthcare workers. In addition to the pediatric wards where the work began, their services have expanded significantly to hospice wards, care centers for older adults with dementia, and even refugee camps and post-disaster psychological recovery areas. Around the world, this art-based companionship continues to bring laughter and warmth into healthcare spaces, making “whole-person care” not just an ideal, but an everyday reality.

Red Nose Doctors in Taiwan

Children are not “smaller versions” of adults. Their physical conditions and emotional needs require healthcare that differs from that for adults.

In 2014, Red Nose Doctor founder Ms. Ma Chao-Chi and Mr. Luc Ducros (Gu Le-Xi) completed training with Le Rire Médecin in France and received French government certification, officially becoming Red Nose Doctors. The following year, the two returned to Taiwan together, put on their makeup, brought their instruments, and entered the wards of National Taiwan University Children’s Hospital. It was the first day that Taiwan’s first clown doctor team entered a hospital.

During this period, in addition to continuing to perform on the front lines of hospitals, Ms. Ma Chao-Chi brought the professional training courses she had studied in France to Taiwan. In 2017, she established the Red Nose Caring Clown Association of the Republic of China. Through lectures, interviews, workshops, and other activities, the Association promoted the philosophy and stories of Red Nose Doctors and actively advocated the importance of child-friendly healthcare, hoping to help more people understand the work of Red Nose Doctors.
